Short Text: Parent address AND student address are missing.

Long Text: Parent address and student address are missing. Please submit either a parent address or a student address so that the Department of Health Services (DHS) can offer summer EBT benefits to the family at the end of the school year.

 

How to Fix: This warning displays when an LEA participating in the National School Lunch Program (NSLP) and a student eligible for free or reduced price meals does not have an address available in the students demographic and/or the parent(s)/guardian(s) information. In cases where the parent has opted out of address sharing, but the LEA is receiving this warning, the LEA can acknowledge the warning and proceed accordingly. NOTE: CEP schools must submit this information as well.

If the family is homeless and there is no address, or if the family is refusing to provide the address due to safety concerns, the LEA should reach out to the Department of Health Services (DHS) [email] or the DPI Homeless Liaisons.

Example: Yolanda, a student being raised by a single mom, is a student at West Middle School. Sadly, Yolanda’s mother recently passed away and her Aunt Pauline is now her legal Guardian. When Yolanda moved out of the home she lived in with her mother, the address was deleted from the West Middle School Student Information System (SIS). District Deb looks to see who Yolanda’s emergency contacts are, and Aunt Pauline was one of them. Aunt Pauline’s phone number was listed, so Deb gives her a call. Aunt Pauline provides Deb with an address for Yolanda, and Deb enters this into the SIS. 

Example: Mark is a student at North Middle School. Mark and his dad currently live in a motel. Mark's grandmother lives within the school district boundaries, but Mark's dad doesn't feel comfortable listing that address due to some safety concerns with other family members. This lack of permanent address triggers warning 7471 for Mark. District Deb confirms that Mark is eligible for summer EBT benefits, and also knows why there is no address listed. Deb reaches out to the Department of Health Services (DHS) with an email and a phone call. Eventually, Deb is able to connect the helpful satff member at DHS with Mark's dad so they can work out the requirements for Mark's summer EBT.

Business Rule Logic:  Throw at Student Level on SEOA if:

  • NSLP School = Yes AND 

  • DPIEconStatus calc =Free or Reduced  AND 

  • (/parent is NOT submitted OR Address Null on /parent) AND 

  • address is null on SEOA
